The easiest way to enroll is to complete the form below, then give us a call at 719 632 5173 to pay the deposit by Visa, Mastercard, or Discover. Be sure to click the "CREATE FORM FOR PRINTING" button at the bottom, because when you do we'll receive an email copy of the form and have your info readily available when you call. Alternately, after contacting us to confirm availability of your desired date(s), the form can be printed and mailed along with a check or money order for the deposit.
